Description
Prior to completing a DDAZ Foundation Program Grant Application, you must register your organization. Please click the green Register box at the top right of your screen.
After you register, please review requirements below:
- Applicant must be a 501(c)3 nonprofit in good standing with the Arizona Corporation Commission, public school, federally recognized Indian tribe or government entity.
- Funds must be allocated for a program (not general operating).
- Programs must fall in the following areas:
- Dental disease prevention
- Oral health education
- Dental treatment
- Medical-dental integration
- Food insecurity and access, nutritional programs
- Organizations may submit only one proposal per grant cycle.
- Programs must benefit underserved, uninsured populations (all ages).
- DDAZ Foundation favors grant requests that demonstrate:
- Operational and financial stability
- Reporting of measurable, meaningful outcomes
- Active collaboration (when appropriate) among organizations, schools, and dental and/or healthcare providers
- Inclusion of diverse groups
- Implementation of referral protocols that link individuals needing dental care with a source of dental care
- Initiation of dental sealant programs into existing oral health programs, particularly in counties where none exist
- Submission of timely and complete Interim and Final reports, if previously funded
- DDAZ Foundation does not fund:
- Individuals
- Sponsorship or attendance at conferences
- Religious activities
- Reduction of organizational debt
- Direct lobbying or influencing of elections
- Duplication of existing services
- Fundraising campaigns and expenses
